All ACMA fees are listed in this section. Other than the fees listed below, there is no other charge at ACMA. For each order, the total ACMA order amount is composed of the following three fees: ACMA Order Amount = Consultation Fee + Treatment Fee + Shipping and Handling Fee. (1) Consultation Fee A consultation includes, but not limited to, the following diagnostic processes each time in preparing for a new ACMA treatment: visit, phone, email, etc. A consultation fee applies every time a new ACMA treatment preparation is made. The consultation fees are indicated by the ACMA codes beginning with "C". For each patient, each time only one consultation code (either C00001 or C00002 below) and corresponding consultation fee is applicable. (2) Treatment Fee A treatment fee applies every time a new ACMA treatment is provided. The treatment fees are indicated by the ACMA codes beginning with "T". For each patient, each time only one treatment code (either T00001 or T00002 below) and corresponding treatment fee is applicable. The listed treatment fee is for one week. The total treatment fee for N weeks will be equal to "weekly treatment fee multiplied by N" (N = 2, 3, or 4). (3) Shipping and Handling Fee The shipping and handling fee is applicable only when the patient requests to mail the ACMA treatment herbs to the patient. If the patient picks up the ACMA treatment herbs, there will be no shipping and handling fee. The shipping and handling fees are indicated by the ACMA codes beginning with "S". If a patient requests to mail the ACMA treatment herbs to him/her, there will be one shipping and handling code (either S00001 or S00002) and corresponding fee applicable to the patient.
